Along with completing the following form, please email five (5) high resolution image and/or video samples or 2-3 writing samples (not to exceed 5,000 words total) to art@faylib.org using the subject line: "Visiting Artist application".

Scoring

+ Technical Skill: Works submitted demonstrate the artist’s skill and proficiency. (10 points)

+ Professionalism: Proposals are organized, efficiently demonstrating the applicant’s expertise in art and instruction. (10 points)

+ Class & Workshop Proposal: Workshop proposals are well structured, including titles, class descriptions, intended audience and learning objectives. (20 points)

+ Originality & Innovation: Projects and workshop proposals will provide unique opportunities and experience to library users. (10 points)
